As part of the pre-conference workshop “Graft Extraction Masterclass,” Dr. Watts presented on the difficulties encountered while performing Non-Shaven Long Hair Follicular Unit Extraction. He emphasized that while the technique is increasingly sought after for its discreet recovery and immediate cosmetic preview, surgeons must navigate a unique set of technical and procedural challenges to achieve consistent outcomes.
Dr. Watts was among the select invited international faculty and also joined the Roundtable Discussion: Development and Prospects of Hair Medicine from a Global Perspective.
Drawing on his own surgical experience, he explained that mastering the technique involves more than new equipment — surgeons must adapt their workflow, unlearn conventional extraction habits, build confidence in machine-assisted systems, and progress through structured case training. He detailed challenges including graft visualisation under long hair, maintaining a clear surgical field, bleeding control, hair matting and entanglement, time management, graft dislodgment, and preserving correct implantation angle, curl, and direction.

He also addressed advanced extraction systems combining rotation, oscillation, and vibration, while stressing that surgeon judgment and understanding of tissue behaviour remain critical. On post-operative care, he noted patients should be counselled about temporary shedding before regrowth and advised on measures such as saline sprays and avoiding early combing. For surgeons adopting the technique, he recommended starting small — within scar repairs or limited patches — before scaling up.
